The callback form asks for your own name, email address, phone number, preferred contact method, preferred callback time and voicemail preference. There is no therapy-interest question, age/child information, general-message box or upload. Do not enter diagnoses, medical records, birth dates or health details. Discuss your questions directly with Shaela. Preferences do not guarantee a particular time, and submitting a request does not book an appointment.

When connected, Web3Forms processes these contact details and preferences and emails the request to the practice's confirmed address. It also processes a request identifier and technical information used to deliver and protect the form. Its current privacy policy describes stored submissions with retention up to three years, infrastructure providers, and spam-prevention services that may receive submitter IP/email information. Acceptance by the service is not proof of arrival in an inbox.

The website has no visitor accounts, clinical-record uploads, appointment booking, online payments, advertising, tracking pixels or third-party application analytics. Fonts and artwork are served with the website. It does not set application cookies. Web3Forms' server-side spam filter is enabled; CAPTCHA is not currently configured. The form service's technical processing is separate from the static website.
